Part of working on the HarmoniMD project for MedWave, I developed an Electronic Clinical Document (ECD) XML structure for collecting data outside of capturing vitals, diagnoses, orders and procedurces. These workflow based documents can be taylored to fit different profession specialties, so the data captured in structured in a meaningful way. While these ECDs were quite easy to hand code, our clients were requesting more and increasingly complex documents conversion. This task to convert paper protocols, progress notes, admissions document and others became increasingly time consuming.

I proposed creating an in-house tool, so creating and editing our ECDs would take minutes, insteading of hours. Now this tool is being used by staff with very limited knowledge of XML. Finished document templates are stored by facility groups while works in progress can be stored and load locally.
There are many features that can be used in these documents such as, tabled data, graphing, repeatable form groups as well as adding vitals, diagnoses, orders, and procedures directly to a facilities EMR database.

There is now a team in Paraguay that works excusively with this application. They are managing workload at multiple hospitals using it to create and maintain documents for their unique workflows.
The Electronic Clinical Document format is currently supported in all of Medwave's business applications and their patient portal as well. In México alone, there are over 3000 unique ECDs being used in several hospitals groups including Hospital FUCAM and Centro Oncológico International, both of which treat thousands of cancer patients.
